The Tremont Neighborhood Health Action Center provides health and wellness services in the Bronx as part of New York City’s effort to promote health equity. Services and activities are held virtually, in-person, or both. Most classes are in English, but if language services are needed, contact the center before the class. Mondays

  • Insurance Enrollment Counselors: 11 a.m. to noon, 1 p.m. to 3 p.m.
  • Vital Records Assistance: 8:30 a.m. to 4:30 p.m.
    • Receive assistance with requesting and correcting NYC birth and death certificates.
  • Immunization Records: 8:30 a.m. to 4:30 p.m.
    • Obtain immunization records or schedule vaccine appointments at various locations.
  • Dog Licensing: 8:30 a.m. to 4:30 p.m.
    • Receive assistance with required dog license applications and purchasing and renewing dog tags.
  • NYC Business Solutions and Workforce Training Connections: 8:30 a.m. to 4:30 p.m.

Blood Pressure Kiosk

There is a free self-serve blood pressure kiosk in the lobby of the Action Center. Anyone can use the kiosk to check their blood pressure when the building is open. Learn more about high blood pressure.

Family Wellness Suite

Family Wellness Suites are safe, welcoming, and supportive spaces for family members to receive services, health education, and connections to infant and maternal health community resources.

Lactation Room

This room provides a safe and comfortable space for parents to breastfeed or nurse their babies. Visit the Family Wellness Suite in the lobby of the Action Center to learn more.

Free Pest Control Service

If a child in your home has asthma, and if you have a problem with roaches or mice, we may be able to help you get rid of them safely using non-toxic methods. Pest prevention education and cleaning supplies are included. For more information contact ipm@health.nyc.gov

Tobacco Treatment Support

Adults 18 years or older who smoke or vape and want to quit or cut back may be able to get:

  • An appointment for a brief coaching session
  • Educational materials
  • A free one-month supply of nicotine patches and lozenges to help them quit, cut back, or avoid smoking and vaping when they want to

Food and Nutrition Programs

Participate in our food and nutrition programming which includes cooking demonstrations, nutrition workshops, healthy cooking classes, and pop-up farmers markets.

Learn about gestational diabetes and food preparation to support participants in having a healthy pregnancy journey.

Weekly Services and Activities

Crib and Car Seat Distribution

Families can request a car seat or safe sleep surface for their infant.

  • Wednesdays, by appointment only
  • Car Seats: Must be at least 27 weeks pregnant.
  • Safe Sleep Surface: For infants newborn to 6 months.

Health Insurance Enrollment Counselors

Health Insurance Enrollment Counselors are onsite Mondays, Wednesdays, and Thursdays from 11 a.m. to 3 p.m. and can assist you with enrolling and renewing your health insurance. Call (646) 942-1469 to schedule an appointment or get virtual assistance. You can also walk in during the above times to get help or submit a Health Insurance Inquiry Form and a counselor will contact you.
