TLC Driver License

Please note: The Drug Test fee is $34.00.

The TLC Driver License is a single license that lets you drive a yellow taxi, green taxi, livery, black car, and limousine. Most drivers get this license, unless they know they are going to drive a commuter van or paratransit vehicle.

Commuter Van Driver License

Commuter vans are vehicles that transport between 9 and 20 passengers in preapproved zones. If you wish to drive these vehicles you must apply for the Commuter Van Driver License.

Paratransit Driver License

Paratransits vehicles provide non-emergency transportation for passengers with disabilities. If you wish to drive these vehicles you must apply for the Paratransit Driver License.

TLC Affirmation for DMV State Change

All TLC Driver License holders & Applicants must complete TLC Affirmation for DMV State Change each time they change their DMV driver license from one state to another state. If you have had (or currently have) a non-NYS DMV license in the past two years, you must submit this form.

Requirement Details

If you change your DMV driver license to a different state (including changing from New York to another state, vice-versa, or changing from any state to any other state), you are required to complete and submit a TLC Affirmation for DMV State Change to the TLC via email to licensingi@tlc.nyc.gov.

This form must be submitted every time your DMV state changes.

How to Submit the Affirmation

1. Complete the Form: Download and complete the required TLC Affirmation for DMV State Change Form.
2. Submit: Email the completed form to the Licensing Inquiry email box at licensingi@tlc.nyc.gov. Please use Subject: TLC Affirmation for DMV State Change form and your TLC  license number.

Important Note

Failure to submit the required TLC Affirmation for DMV State Change Form when you have changed your DMV license state to a new state may negatively impact your TLC License status.
