Provider Eligibility Criteria

To join the NYC VFC program, providers must serve children from birth through 18 years of age who fall under any of the following categories:

  • Medicaid eligible
  • Uninsured
  • Underinsured
  • American Indian or Alaska Native
  • Enrolled in Child Health Plus B (CHPlus B)

Children aged 0 through 18 years that do not meet one or more of the federal vaccine eligibility categories (VFC-eligible), are not eligible to receive VFC-purchased vaccine

In addition to serving VFC eligible populations, providers must NOT be included in the Medicare/Medicaid Restricted and Excluded Individuals or Entities list.

How to Enroll in the VFC Program

  • Providers who serve VFC eligible children can enroll in the VFC Program through the CIR Facility Manager website. First ensure the facility is enrolled with the Citywide Immunization Registry (CIR).
  • After your facility is enrolled and approved, you may proceed to the “Program Enrollments” tab in the CIR Facility Manager and complete the VFC Enrollment Form.
  • Once your enrollment submission is reviewed, you will be contacted by a PQA staff member to schedule a New Enrollment site visit. You will not be able to order vaccines until we conduct a site visit and ensure that your practice meets all storage and handling guidelines.

How to Reenroll in the VFC Program

The VFC program requires providers to reenroll biennially through the the CIRFacility Manager (FACMAN). Providers will receive notices once the online reenrollment form is deployed in FACMAN. Reminders will be sent to remind providers who have not reenrolled. It is important that the contact information we have on file for site is up to date at all times.

  • VFC reenrollment must be completed online through FACMAN biennially. By completing the reenrollment process, providers agree to the VFC program requirements listed in the Provider Agreement (PDF). After completing the re-enrollment form, providers will receive a confirmation that the program received the completed form.
  • The physician-in-charge, vaccine coordinator, and back-up vaccine coordinator at each VFC facility are required to complete the You Call The Shots: Vaccine Storage and Handling and Vaccines For Children (VFC) web-based training courses and submit certificates of completion as a part of reenrollment.
